These Terms & Conditions (“Terms”) govern your access to and use of the website, patient portal, and services provided by Direct Health Primary Care, LLC (“Clinic,” “we,” “our,” or “us”). By accessing or using our website or services, you acknowledge and agree to be bound by these Terms.
1. Informational Use Only
The information contained on our website is provided solely for general informational purposes. Nothing on this site should be construed as medical advice, diagnosis, or treatment. Accessing this site or communicating with us electronically does not create a provider–patient relationship unless and until you have formally registered as a patient, completed intake documentation, and been accepted into our practice.
2. Medical Services
All medical services are provided only after proper registration and acceptance into our practice. We reserve the right, in our sole discretion, to accept or deny patients.
3. Membership & Payment
– Membership fees are billed [monthly/annually], in accordance with the terms set forth in your Membership Agreement.
– All payments are due in advance and are non-refundable unless otherwise required by law.
– Failure to make timely payment may result in suspension or termination of services.
– The Membership Agreement, which is incorporated herein by reference, shall govern additional financial and service-related terms.
4. Telemedicine Services
By utilizing our telemedicine services, you acknowledge and agree that:
– You are responsible for ensuring the accuracy of information you provide.
– There are inherent risks to telehealth, including but not limited to service interruptions, unauthorized access, and technological limitations.
– Telemedicine is not appropriate for all medical conditions, and the determination of suitability shall rest exclusively with the provider.
5. Acceptable Use of Website
You agree not to engage in any conduct that could damage, disable, or impair our website or interfere with another user’s access. Unauthorized attempts to gain access to our systems or data are strictly prohibited and may result in legal action.
6. Limitation of Liability
To the maximum extent permitted by applicable law, Direct Health Primary Care, LLC and its owners, employees, contractors, and affiliates shall not be liable for any direct, indirect, incidental, special, consequential, or punitive damages arising from or related to your use of our website, services, or reliance on any content therein.

9. Governing Law & Jurisdiction
These Terms shall be governed by and construed in accordance with the laws of the State of Georgia, without regard to conflict-of-law provisions. Any disputes shall be brought exclusively in the courts located in Richmond County, Georgia.
