Membership Commitment & Billing Policy
Minimum commitment

First month at 50% prorated

Your first payment includes your first month plus the following two months, collected upfront. After that, your membership automatically renews on a month-to-month basis.
Why 3 months?
It allows Dr. Patel to establish care and provide continuous, meaningful treatment.
Choose the Membership Group applicable for your age group.
- No enrollment fees for new patients!
- Enrollment fees for returning patient who cancelled memberships previously will be $150 per person.
- 30 day notice required for membership cancellations.
